Deposits and Payments

For the convenience of our students, you are able to register and pay for your class in one simple and secure on-line transaction.

Deposits must be paid at the time of registration. The deposit for all classes is $50.00 ($25.00 for CPR when taken without a refresher). Deposits are fully refundable if notice of cancellation is received within 24 hours of the start of class. Afterwards, no refund will be issued. The balance of the course fee must be paid by the time class begins on the first day. The balance must be paid either online using our secure website, in class with a credit card, money order or cash - NO PERSONAL CHECKS will be accepted. Refunds will be given up to 24 hours prior to the start of class with written notice from the student. However, once the class starts there will be no refunds even if the student does not successfully complete the course.

Once registered, students must bring the confirmation receipt and a current CPR and EMT-B certification cards to the first day of class. They will be copied and kept in the student file.

If a student is unable to register online, then they can print out and mail in the registration with the required $50.00 (or $25.00 for CPR alone) deposit. Only Money Orders will be accepted for mail-in registration.

 

 
Attendance and Student Expectations

Attendance: Students attending courses offered by EMS Education, LLC., are expected to arrive on time for class according to the Class Schedule. Students arriving greater than 15 minutes past the class session start time will not be allowed to participate in that class. The student will be given the opportunity to attend a different session covering the same content.

Attire: Proper attire should be worn so as to respect the proper decorum of the classroom setting and not potentially offend the other students and instructor. Unacceptable attire means no “short” shorts or tops which bare the midriff for females. Male students are expected to wear properly fitting clothes and no tank tops. The decision as to whether or not a student is dressed appropriately is at the sole discretion of the Program Director.

Courtesy: Students can expect to be treated courteously and fairly. The staff of EMS Education expects the same in return. Behavior deemed to be disruptive to the class may result in being expelled from the class. The student will first be spoken to concerning their behavior and given an opportunity to correct their behavior. All decisions regarding expulsion from the class are at the sole discretion of the Training Program Director.

Phones, Pagers and Sidebars: In order for all students to have the opportunity to learn, we request that all phones and pagers be put on silent before the start of class, and that students refrain from having sidebar conversations.

 

 

Record Keeping
All course records will be securely stored at our location at 23005 N. 15th Avenue, Suite 203, Phoenix, Arizona, 85027 in compliance with all applicable State, Federal and Privacy laws. EMS Education, LLC., will not share your personal information or class performance with anyone unless authorized to do so by the student. The State of Arizona Department of Health Bureau of EMS and Trauma System may inspect any and all student records held by EMS Education, LLC., for completeness and to ensure compliance Arizona Administrative Code R9-25 Article 3. Further per A.A.C. R9-25-316 (B) EMS Education, LLC., must submit a course roster to the Bureau of EMS and Trauma System within 10 days of the course completion. This roster will include the student’s name, address and social security number which the Bureau of EMS and Trauma System also hold confidentially. This is a requirement of all Training Programs in the State of Arizona.
